2/16/2023 EnchantaVaughn Approved 1. Behavior: Upon arrival, client opened the door for QMHP and went straight to an empty space in the house to begin session. QMHP engaged mother in a discussion to determine behaivors since the last session. Mother reports client continues to not want to listen to those who are in authouity and not take responsibility for his behaviors while placing the blame on others. 2. Behavior: Client demonstrated good manners to QMHP by being courteous and polite. Client engaged in conversation by asking and answering appropriate questions. Client gave good feedback, indicating he was listening and interested in the psycho educational activity. Client needed redirection during the session for blaming his siblings at times for his behaviors in the home as well as in the school setting when he gets in trouble in school. 1. Intervention: QMHP went to the home to meet with client individually. QMHP engaged in conversation with client about his day. QMHP utilize positive self-talk strategies to help client think positively at all times and to talk about his actions and how they will make people feel. QMHP utilized active listening skills while client was verbalizing information about his day. QMHP will review key concepts of being responsible with his family. QMHP educated client about being respectful, encouraging, mindful, and fair to peers and authority while making good decisions in the home. QMHP will provide mediation and relaxation techniques, as well as, simple praises and encouragement to continue the positive behaviors he was exhibiting. QMHP aided this intervention with client on social skills. QMHP introduced client to psycho social activity regarding hygiene and staying healthy. The QMHP educated client that it is vital to wash your hands and stay inside especially during the winter months for the safety of his elders who take care of him. QMHP encouraged client to focus on one task at a time and then he can engage in activities and hobbies of his choice. QMHP assisted client with focusing on one task at a time without exhibiting hostile or aggressive behaviors towards dad and grandmother. QMHP led the family through each treatment exercise successfully. 2. Intervention: QMHP reminded client to be true to himself and reward himself for small things. QMHP discussed with client how important it is to maintain healthy relationships instead of focusing on having a relationship with everybody. QMHP educated to client that everything that everyone does is not always the best example and he should follow his own instincts and judgments. QMHP will continue to assist client with working towards targeted goals and objectives. QMHP role played with the client to assist him in bettering his social-emotional-mental skills. QMHP reviewed the positive things that client had done and encouraged him to continue in making positive decisions. QMHP completed a fill in the blank activity with client to help him identify the difference between a positive and negative response. QMHP urged father and client to participate in role reversal activity to aid father and client to realize how to respond to and interact with each other. QMHP observed client’s behaviors as seemingly calm and controlled as demonstrated by his ability to release his energy appropriately in a social environment without instructions. QMHP completed another activity with client to teach him good mental habits, such as exercising and meditation or yoga. QMHP discussed client’s responses with him and modeled for him how to utilize what he has to rebuild mentally. 1. Response: Client displayed management of individualized behaviors. Client was assisted in practicing general communications skills. Client understood effective problem-solving skills to help solve or overcome a problem or stressor the he is currently facing. Client understood that when he becomes agitated he needs to refrain from taking it out on people and to communicate how he is feeling to others. Client communicated positive affirmations and self-talk once he was able to let go of negative, agitated feelings. Client shared some of the things he does at home to help him relax. Client participated in reviewing key concepts. Client showed good understanding of what it means to be respectful, encouraging, mindful, and fair to others. Client reported a decrease in symptoms following the relaxation techniques. Client was able to share his feelings. Client expressed that he’s trying his best. Client and dad understood the contract for safety and implemented appropriate behaviors, at this time. The client has not overreacted with anger to minor frustrations or irritants. Client responded in a pleasant manner acknowledging the positive reinforcement that the QMHP provided. Client discussed mental illness and its causes as a family. Client displayed interest in learning about ADHD. Client was able to reevaluate himself and display a more positive attitude toward peers and authority. Client demonstrated expectations and responded well to feedback. Client reported a decreased stress level due to his modulation of social contact, and the success of this was reviewed. Response: Client and dad were observed interacting well with one another. Client was receptive and open to the interventions, as he was attentive and focused on the information and activities that were being processed without any difficulties. Client nodded in understanding and communicated that he was going to continue making good choices and show positivity around others. Client was acceptive of extra help and support. Client utilized the resources provided and seem to be doing better with managing symptoms of ADHD. Client recognized positive behavior methods that would be useful in the home and community as a way to develop skills needed to stabilize symptoms associated with ADHD. Client was open and receptive to the intervention presented by the counselor. Client was able to verbalize the concept of the intervention and provided verbal feedback throughout the session. Client actively engaged in the role-playing activity effectively exhibiting the learned calming strategy. Client maintained positive disposition throughout the session and did not need to be redirected or prompted by authority. Client continues to build a healthy relationship with his counselor. Client was receptive to all interventions given by QMHP. Client displayed appropriate understanding of exercise. Client was attentive while counselor assisted him with reviewing his schedule and preparing for the next day. Client was able to recognize his target behaviors and implement a plan to reduce behaviors that were not helping him with his treatment.
